  • Active Secretary of State License (or Active Seller's Permit if Sole Proprietor)
  • Active General Liability Insurance (Minimum of $1,000,000 and Workers' Compensation where required)
  • BBB Rating ("B+" or better)
  • Yelp and/or Google Rating (Minimum of 4.0 or above)
  • Company Name on Application must match all Supporting Documentation (including the use of a DBA if applicable)
